The anabolic effect of combining salbutamol with L-dopa

Ten years ago neurologists at the University of Arkansas published in Clinical Neuropharmacology the results of a small study, which may or may not be of interest to our dear readers. Nor do we know for sure whether salbutamol and L-dopa – the combination that the neurologists tested in the study – is completely safe. What we do know is that the anabolic effects that the researchers observed are remarkable.

Nor do we know for sure whether salbutamol and L-dopa – the combination that the neurologists tested in the study – is completely safe. What we do know is that the anabolic effects that the researchers observed are remarkable.
The eight people who served as subjects for the neurologists’ experiment all suffered from Parkinson’s and had an average age of 59. They were already taking L-dopa, an amino acid that boosts the production of dopamine in the brain. The body’s production of dopamine declines rapidly with the onset of Parkinson’s and as a result brain cells lose their function. L-Dopa supplementation reduces the symptoms in the first stage of the disease.
The average L-dopa dose of the subjects was 481 mg per day.
Cells absorb more L-dopa, and therefore also produce more dopamine, when they are exposed to adreno-beta-2-agonists. That’s why the researchers wanted to try the combination of salbutamol, a mild adreno-beta-2-agonist, and L-dopa, to see what effect it would have on their patients. They gave their subjects 4 oral doses of 4 mg salbutamol daily.
In one sixty-year-old patient the salbutamol/L-dopa combination caused headache, restlessness and trembling. She stopped taking the combo after two weeks. The other subjects continued for 14 weeks. During this period they also did strength training twice a week.
The table below shows that the subjects’ maximal strength increased by a modest amount and that their body composition changed considerably. While there was no change in the subjects’ bodyweight, their lean body mass increased by 7.7 kg.

Don’t take any notice of the weight that the subjects lifted with the chest press. No doubt there’s an editorial error there.
In half of the patients the addition of salbutamol to the L-dopa medication reduced the symptoms of the Parkinson’s.
The researchers emphasise that they don’t know whether the combination they tested is safe.
“The observed significant increase in the mean heart rate, seen in ten percent of patients using salbutamol, and the isolated case reports of cardiac arrhythmias and angina underscore the need for careful cardiac evaluation when use of salbutamol is considered”, they write. “The central nervous system side effects include enhanced physiologic tremor (twenty percent), insomnia, dizziness, drowsiness, restlessness, irritability, nervousness, visual hallucinations, and headache.”

Albuterol improves response to levodopa and increases skeletal muscle mass in patients with fluctuating Parkinson disease.

Abstract
Animal studies indicate that beta(2)-adrenergic receptor agonists enhance transport of levodopa across the blood-brain barrier. Preliminary studies showed improved response to levodopa in patients with Parkinson disease (PD) who were given albuterol as adjunctive therapy. Beta(2)-adrenergic agonists may offer additional benefits to PD patients via their skeletal muscle anabolic effects, particularly those who experience decreased muscle strength and weight loss. Nondemented, fluctuating PD patients receiving levodopa but not experiencing severe dyskinesias underwent the following tests at baseline and 14 weeks after treatment with albuterol sulfate (4 mg four times a day, orally): Unified Parkinson’s Disease Rating Scale motor, tapping, and stand-walk-sit tests every 30 minutes between 8 am and 5 pm; body composition analyses using whole-body plethysmography and computed tomography of the thigh; muscle strength tests; and the Parkinson’s Disease Questionnaire (PDQ-39). Results were analyzed using paired t-tests (2 tailed), repeated-measures analysis of variance, and the Wilcoxon signed-rank test. Seven of 8 enrolled patients completed the study; 1 patient withdrew because of headache and anxiety. The area under the curve for all-day UPDRS motor scores improved by 9.8 +/- 9.1% (mean +/- standard deviation; P < 0.05) and tapping improved by 7.6 +/- 8.1% (P < 0.05). The effect was more pronounced when only the response to the first levodopa dose (area under the curve, 8-11 am) was analyzed: 13.0 +/- 9.8% and 9.8 +/- 9.6% respectively. Thigh muscle cross-sectional area increased significantly as measured by computed tomography (5.3 +/- 3.2%, P < 0.01), as did fat-free mass by whole-body plethysmography combined with total-body water determination (9.5 +/- 2.9%, P < 0.05). There was no significant improvement in the stand-walk-sit test, muscle strength tests, other UPDRS sections, daily OFF time, or PDQ-39. Four patients were rated as having a mild global improvement (+1 point) on a -3 to +3-point scale, and 3 of them chose to continue albuterol beyond the termination of the study. The mean heart rate increased from 78.3 +/- 9.3 beats/minute to 85.6 +/- 8.7 beats/minute (P < 0.05). No laboratory abnormalities or electrocardiographic changes were induced by albuterol in any subject. This open-label pilot study suggests that albuterol increases muscle mass and improves the therapeutic response to levodopa in patients with fluctuating PD. A double-blind, placebo-controlled study is needed to confirm the effects and safety profile of beta(2)-agonists in PD.

Do they really equal more energy?

The sunny days of summer have passed and winter blah begins. This time of year many find it a challenge to make it out of the door for a daily run. Lunchroom conversation often turns to a report about a lack of energy and motivation to keep up with the daily routine. In an effort to manage the necessities of life or to look for an extra boost, many resort to popular energy drinks such as Red Bull, Rockstar, and Monster.
Do energy drinks provide more energy? To answer this question it is important to evaluate the ingredients in an energy drink. Most consist of high doses of caffeine and other stimulants such as ginseng, ginko, guarana, and taurine. They also contain added minerals and vitamins such as B Vitamins. Lastly, they often contain high doses of sugar.

In breaking down the ingredients let us first look at caffeine. Although the amount of caffeine is not indicated on many energy drink labels, the average energy drink contains about the same amount of caffeine as three cans of cola. The added caffeine can, of course, provide a greater sense of alertness. However, caffeine highs are often followed by lows. This may lead to an erratic sense of energy over the course of the day.

Some may respond to the high dose of caffeine feeling a sense of nervousness, anxiety, increased heart rate and blood pressure, bowel irritability, as well as having irregular sleep patterns. It is also important to keep in mind that caffeine is addictive. If daily consumption of an energy drink is eliminated or reduced, one is likely to suffer withdrawal symptoms. Lastly, the caffeine content has the potential to be dehydrating.

The purpose of the herbal stimulants in energy drinks is to enhance the effect of caffeine. Unfortunately the combination of stimulants with caffeine has not been studied enough to define it as safe. Although each ingredient individually may appear to have potential health advantages, it has been questioned if the ingredient combination contributes health risks for those with seizure activity and heart related conditions. This may be disconcerting since we can be unaware of underlying medical condition. At this point there is no evidence to support short or long term benefits of the added stimulants.

The drinks are often fortified with vitamins and minerals giving the appearance of added health benefits. There is no major disadvantage to safe levels of fortification. However, the recommended intake of the added nutrients can be met by simply consuming a healthy diet.

The high sugar content cannot be overlooked. A high dose of sugar commonly provides the body with a boost of energy. Some claim that the sugar content of such energy drinks is contributing to the obesity epidemic - particularly among youth since that is where marketing is targeted. Energy in the form of sugar is often short lived and not worth the calories. 
The attraction to energy drinks as a fun alternative to a bottle of water is understandable. If you choose to drink such beverages be sure to read and compare labels carefully. It is wise to pay particular attention to the content of caffeine, herbal stimulants, and sugar as well as the number of servings per container.

There are a few energy drinks on the market that may have a place in a healthy diet. For example, Airforce Nutrisoda provides all natural energy type beverages. Most of the beverages contain no sugar, caffeine, sodium, or aspartame. They are fortified with vitamins and minerals that are all at safe levels and cans are clearly labeled with the exact content in each soda.

In the debate over whether to choose an energy drink, the cons of the beverages often outweigh the pros. A natural, healthy diet and lifestyle can provide much more sustained energy than a drink loaded with caffeine and sugar. Remember, an energy drink should never be confused or supplemented for sports beverages such as Gatorade during exercise. A balanced, healthy diet with a regimen of adequate sleep is truly the best and safest approach in providing yourself with sufficient daily energy.

Caffeine and running performance

Caffeine is a ubiquitous drug that you almost undoubtedly consume in coffee, soft drinks, tea or chocolate. Caffeine has a wide variety of effects on the body, some of which may enhance running performance. But, like any drug, caffeine also has side effects, and some of them can be detrimental, particularly in high doses.
Can caffeine help you race better? Several hundred studies have been conducted on the effects of caffeine on sport performance, and the answer is: probably. Studies with runners and cyclists have shown that caffeine can improve performance in the lab during simulated events lasting from five minutes to several hours. Extrapolating from run-to-exhaustion studies in the lab, the likely benefit of taking caffeine is in the range of 1 to 2 percent (20 to 50 seconds in a 10K, 90 seconds to four minutes in a marathon).
Hardly any studies have investigated the effects of caffeine on race performance, however, so we do not really know if it can help improve your next 10K or marathon. One reason that caffeine may not work as well during races as in lab tests is that caffeine increases epinephrine (adrenaline) levels, which are also stimulated by the excitement of competition, so the caffeine may be redundant during a race.
How does caffeine affect running performance? Despite all the research, no one is sure. The conventional wisdom is that caffeine improves endurance performance by increasing the activity of enzymes for fat metabolism. By using relatively more fat, the runner’s glycogen stores are used more slowly, so they last longer. This theory is under question, however, because caffeine also improves performance in events that last only a few minutes, in which glycogen depletion is not a factor.
The primary effect of caffeine in improving endurance performance may be stimulation of the central nervous system, which increases alertness and concentration. There is intriguing evidence that central nervous system stimulation reduces perception of effort so that a given pace feels easier. Determining how caffeine improves running performance is difficult because the metabolism of caffeine in the body is complicated, with caffeine quickly broken down into three other compounds that have a variety of effects on the mind and body.
How much caffeine is required to enhance running performance?Most of the studies that have found caffeine to improve endurance performance used 3 to 6 mg of caffeine per kg body weight taken one hour before exercise (for a 147-pound [67-kg] runner, 3 mg per kg body weight would require 200 mg of caffeine). Recent studies have also found that ingesting 1 to 2 mg per kg body weight has a positive result. Doses above 6 mg per kg body weight have increased negative side effects and do not lead to greater improvements in performance.








Coffee contains many chemicals other than caffeine, and a 1998 study found that drinking coffee did not have the same performance-enhancing effects as pure caffeine. Energy drinks are not a good choice because they typically contain many other substances and may not be readily absorbed. If you choose to take caffeine for performance enhancement, it is probably better to take a caffeine tablet so you can regulate the dose more precisely.

What are the risks? As of January 2004, caffeine is no longer a restricted drug by the World Anti-Doping Agency. There are risks, however, in taking caffeine before a race. The side effects include headaches, dizziness, anxiety, gastrointestinal distress, and heart palpitations. Caffeine is also a mild laxative, which can be particularly inconvenient during a race.
The side effect that is most likely to reduce running performance is caffeine’s diuretic (urine producing) effect. Caffeine may increase dehydration, which would cancel out its performance-enhancing benefits. Interestingly, taking caffeine during exercise does not seem to increase urine formation, and caffeine is less of a diuretic in individuals who are regular caffeine users.
The positives and negatives of caffeine vary between individuals. A dose that has few side effects and improves performance for one runner may cause marked side effects and reduce performance in you (and the effects may even vary from day-to-day). If you have any medical conditions, such as high blood pressure, check with your doctor before using caffeine.

My view is that runners should only consider using caffeine if they are already training hard and intelligently, have an excellent diet, and are working to optimize various other aspects of their lifestyle. If you decide to try caffeine to improve your race performance, start with a low dose and do not experiment in an important race.

Наряду с этим было показано, что активация гликогенфосфорилазы и гормонзависимой липазы наблюдается только в случае введения в организм таких пациентов адреналина в количествах, позволяющих имитировать изменения уровня этого катехоламина, происходящие в организме здорового человека при выполнении физических упражнений. Это свидетельствует о роли адреналина в активации гликогенолитических и липолитических путей, а также о том, что под его влиянием отмечается параллельная активация внутримышечного расщепления триглицеридов и гликогена, и дальнейший выбор субстрата для процессов энергетического обмена происходит в мышце па другом уровне (Kjaer et al., 2000).
У лиц с поврежденным спинным мозгом наблюдается утрата произвольного контроля над нижними конечностями, а также отсутствует обратная связь между мышцами и соответствующими центрами головного мозга. Разработка соответствующего оборудования позволила таким людям выполнять с электрической стимуляцией функциональные упражнения на эргометре, которые сопровождаются ростом потребления кислорода до 1,0—1,5 л-мин'1. Благодаря этому появилась возможность для изучения метаболизма углеводов и жиров, а также метаболических изменений при выполнении физических упражнений. Применение в качестве средства воздействия вынужденных физических упражнений у лиц с поврежденным спинным мозгом позволило показать, что при отсутствии двигательного контроля и обратной связи мышц с ЦНС наблюдается нарушение образования глюкозы в печени путем гликогенолиза, что приводит к постеленному снижению уровня глюкозы в крови во время выполнения упражнении (Kjaer et al., 1996). Вместе с тем у здоровых людей при параличе, вызванном эпидуральной блокадой, также происходит нарушение процессов мобилизации глюкозы из печени (Kjaer et al., 1998). Более того, у лиц с травмой спинного мозга во время выполнения упражнений руками (на эргометре для рук) сохраняется состояние эугликемии. Симпатоадренергическая активность и метаболизм жиров

Внутривенное введение адреналина в состоянии покоя вызывает усиление липолитической активности, оцениваемой путем микродиализа образцов подкожной жировой ткани, и этот эффект постепенно ослабевает при повторных инъекциях адреналина (Stallknecht, 2003). У пациентов с травмой спинного мозга во время выполнения упражнения на эргометре для рук методом микродиализа проводили определение уровня липолиза в образцах подкожной жировой ткани, взятых в областях выше и ниже границы, разделяющей область тела, имеющую симпатическую иннервацию (в пределах ключицы), от области се лишенной (над ягодицами) (Stallknecht et al., 2001). В обоих областях при выполнении физических упражнений наблюдалось возрастание интенсивности липолиза, что говорит о том, что прямая симпатическая иннервация не имеет особо важного значения для процессов липолиза при выполнении мышечной работы. Это может означать, что адренергическая активность приводит к одновременной мобилизации внутримышечных запасов гликогена и триглицеридов, а дальнейший выбор субстрата для процессов энергообеспечения осуществляется на другом уровне.

Testosterone therapy improves sex drive and erection capacity in men with low blood levels of the hormone (less than 300 ng/100 ml). A Harvard University study by Ernani Rhoden and Abraham Morgentaler showed that testosterone supplements increased erection capacity, sex drive, energy, and mood in nearly two-thirds of men during a 12-month study. The men received either testosterone injections or testosterone gel.
Testosterone did not improve sexual function in 26 percent of the men. Most middle-aged and older men with low testosterone levels benefit from testosterone supplements within three months and can tolerate treatment for at least a year. (Journal of Sexual Medicine, 7: 277-283)

Теоретическое обоснование звучит так: кофеин стимулирует секрецию эпинефрина - гормона, который "извлекает" жиры из жировых клеток, где они хранятся, и дальше эти жиры становятся топливом энергообмена.

Однако этот эффект, по-видимому, зависит от формы, в которой кофеин поступает внутрь организма. По крайней мере, у тех, кто регулярно пьет кофе, ничего подобного обнаружено не было. Может быть, все дело в том, что кофе обычно пьют со сладким печеньем или пирожными - углеводы "убивают" действие кофеина напрочь.

В бодибилдинге кофеин результативен, поскольку он усиливает поступление кальция внутрь мышечных клеток, а это реально повышает мышечную силу. Помимо этого, для культуристов, страдающих от монотонности тренинга, важен бодрящий эффект кофеина. Он вызывает состояние психо-эмоционального подъема.

Как оказалось, кофеин стимулирует образование в печени особой разновидности белка крови, который впрямую связан с тестостероном. Этот белок производит "захват" молекул тестостерона и делает их неактивными. Кстати, эту работу проделывает и другой белок крови - альбумин. В целом оба белка "обезвреживают" до 98-99% тестостерона, секретируемого половыми железами. Таков защитный механизм природы, оберегающий клетки от раковых мутаций под действием андрогенных и анаболических свойств тестостерона. Только один или два процента молекул тестостерона остаются "свободными". Они то и взаимодействуют с мышечными клетками, вызывая их рост.

Известно, что уровень белков в крови снижают такие гормоны, как гормон роста, инсулин и андрогенные стероиды, а повышают женский половой гормон эстроген и гормоны щитовидной железы. Теперь последний список дополнил еще и кофеин.

Между тем, многие специалисты отказались принять данные последних экспериментов на веру. Дело в том, что участниками исследований были пожилые женщины. Похоже, что их организм устроен иначе, чем у людей среднего лет. Во всяком случае, когда они принимают бор, это повышает у них уровень тестостерона в крови. Когда-то это послужило основанием для производства пищевых добавок с бором, но как оказалось, эти добавки не дают такого эффекта у молодых людей. Возможно, что с кофеином повторяется та же история.

В любом случае, вы должны знать, что кофеин способен "пережигать" жир, но имеет целый букет отрицательных побочных последствий. И если вздумаете использовать кофе как стимулятор, знайте, что большие количества обыкновенного кофе легальны. В случае, если вы одолеете за раз 125 стаканов чая или 75 кружек кофе, вы тут же скончаетесь на месте от удушья или остановки сердца.

Бесспорным является эффект кофеина. Он реально усиливает метаболизм, что сопровождается "сжигаем" жира. Только одна чашка кофе, содержащая обычно 100 мг кофеина, повышает метаболизм на 3-4 процента в течение 2 часов. А вот если выпивать по чашке кофе через два часа в течение 12 часов, то метаболизм повысится уже на 8-11 процентов, а это уже режим "сжигания" жира.
Термогенный эффект кофеина повышают упражнения, поэтому кофе рекомендуют пить перед тренировкой. К тому же, действие кофеина можно еще больше усилить, если принять его вместе с эфедрином и аспирином.

Между тем, ученые подчеркивают, что лучше всего применять кофеин в чистом виде. Такой кофеин входит в состав многих "сжигателей" жира, наравне с эфедрином.

Обычно кофе имеет либо минимальный термогенный эффект или совсем его не имеет. Канадские исследователи обнаружили в распространенных сортах неизвестное вещество, блокирующее эффект кофеина.

Для культуристов крайне важно то, что прием кофеина может нейтрализовать действие креатиновых добавок.
